What Are Supported Living Services (SLS)?



Supported Living Services (SLS) help adults with developmental disabilities live in the home and community of their choice while building the skills needed to live as independently as possible.


Every person's goals are different. Some participants need hands-on assistance with daily tasks, while others want coaching to build new skills and increase independence. Most participants receive a combination of both.

What Can SLS Help Me With?

  

Our staff provide individualized support based on your goals and needs, including:

  • Daily living skills (cooking, cleaning, laundry, hygiene)
  • Meal planning and grocery shopping
  • Health self-management and appointment scheduling
  • Budgeting and money management
  • Public transportation and community access
  • Housing and independent living skills
  • Building healthy relationships and social skills
  • Self-advocacy and understanding your rights
  • Recreation, wellness, and community participation


Services are individualized through your Individual Program Plan (IPP) and are designed to help you become as independent as possible.

What's the Difference Between Assistance and Skill Building?

Sometimes you simply need someone to help. 


Other times, you want to learn how to do something yourself. 


At Annex Initiative, we provide both.


PERSONAL ASSISTANCE

When needed, staff provide direct support to help ensure your health, safety, and daily functioning. This may include assistance with personal care, meal preparation, household tasks, transportation, and other activities of daily living.


TRAINING & HABILITATION (The Heart of SLS)

Our primary goal is to help you build lasting skills. Staff teach through coaching, modeling, practice, repetition, and gradual reduction of support so you can become more independent over time.


Our goal isn't just to help you today  —  it's to help you succeed tomorrow.

What We Don't Do

 To help clarify our role, Annex Initiative does not provide:


  • Skilled nursing or medical treatment
  • Representative payee services or management of participant funds
  • Housing ownership or financial interest in participant housing


Instead, we help participants learn to manage these areas independently while connecting them with appropriate community resources when needed.
