Supported Living Services (SLS) help adults with developmental disabilities live in the home and community of their choice while building the skills needed to live as independently as possible.
Every person's goals are different. Some participants need hands-on assistance with daily tasks, while others want coaching to build new skills and increase independence. Most participants receive a combination of both.
Our staff provide individualized support based on your goals and needs, including:
Services are individualized through your Individual Program Plan (IPP) and are designed to help you become as independent as possible.
Sometimes you simply need someone to help.
Other times, you want to learn how to do something yourself.
At Annex Initiative, we provide both.
When needed, staff provide direct support to help ensure your health, safety, and daily functioning. This may include assistance with personal care, meal preparation, household tasks, transportation, and other activities of daily living.
Our primary goal is to help you build lasting skills. Staff teach through coaching, modeling, practice, repetition, and gradual reduction of support so you can become more independent over time.
